What to expect when coming to Monarch

Once your first appointment has been scheduled, you will receive a confirmation email with all details you need to know, this includes documents you need to complete and return prior to your visit. Forms include:

  • Consent to treatment 
  • Consent to financial agreement 
  • Emergency contact details 
  • Record release form

What we require from you. 

  • A clear copy of your passport and Emirates ID
  • Any relevant medical reports including bloods and radiology reports
  • ·Current medication.  

If you are uncomfortable filling in any forms online, you can fill in when you come in in person.  


You will check in at reception, with your Emirates ID or passport, the clerk staff will ask you to confirm your contact details before fully registering you. 

If you have not filled paperwork sent to you via email, you will be asked to fill out required paperwork. 


Nurse assessment.  

Once you are done with registration, the nurse will call you in for a comprehensive assessment, to build up an accurate picture of your needs.  The nurse will check your vitals, (blood pressure, heart rate, height, weight, etc.) and ask you questions including:  

  • Reason for visit, 
  • Your symptoms, 
  • Medical and social history, including education, relationships, living situation and career.



The initial consultation will typically be about an hour and half long, this time is for you and the clinician to get to know each other, the clinician will go though a detailed intake, to try and formulate a therapeutic treatment plan for you. 


You only have to talk about what you want to talk about. We encourage for you to bring in whatever you need to feel comfortable.   


The outcome of the assessment will be discussed with you, and you will be involved in the decision making about what treatment is best for you.

Take the opportunity to ask any questions about your condition, the diagnosis, treatment options.


You might be given information to take home, and tips for additional research.

Before your appointment, we encourage you take the time to make some notes about what you want to discuss. 


Do not be afraid to ask any question, if anything is unclear to you, let the clinician explain it to you until you are sure you understand it. Repeatedly if necessary.   


The length of treatment is individualized, it will vary according to your needs. It will depend on your condition, issues and goals,  Your clinician will work with you to come with best treatment plan for you. 


Monarch Health Centers takes strict measures to protect your confidentiality, and privacy, regarding your care, information, and records. 

All staff at Monarch Health Centers are trained to protect your privacy. Staff have to uphold and abide by policies and procedures surrounding protection of each patient confidentiality.  Limits to confidentiality exist at Monarch health Centers, and Staff will be  mandated to disclose information under the following circumstances:

  • Imminent risk of harm to self or others.
  • Reported or suspected abuse or neglect of a child, elderly, or disabled individual.
  • Files are demanded by the Courts for legal purses, e.g., court case involving patient.


 

  • Consent for treatment is  required from the legal guardian of children under 18 years of age. 
  • Depending on the concern for visit and the age of your child, the clinician will first meet with child and parents together for initial consultation to get some background information about the concerns that your child is facing along with some developmental history. The sessions might be broken down to three sections,

  1. Meeting the clinician with child and parent. 
  2. Meeting the child alone.  
  3. Meeting with parents alone.

The timing for each meeting during initial consultation may differ.   

  • If your child is between 13 to 18 years, the case might differ, the consultation might just be with child alone, before parents involvement. 
  • The clinician will break confidentiality if they feel that your child is at risk and could be a  danger to themselves or to others.
